Atlantic Physical Therapy Center will make every possible effort to schedule your initial evaluation as soon as possible.  Please call any of our locations to schedule your appointment.  To expedite scheduling your appointment, please have the following information readily available:

1. Insurance Company Name
2. Affected Body Part
3. Dr. prescribing the therapy

You should plan on arriving about 5 minutes before your scheduled appointment time to allow time for your patient registration.

To expedite the registration process for your first visit at Atlantic Physical Therapy Center, please print and fill out the attached form and bring it with you.
